Submitting a DMCA Notice of Infringement
The Digital Millennium Copyright Act specifies that all infringement claims must be submitted in writing (either by electronic mail or paper letter) and must include the following:
- A physical or electronic signature.
- Identification of the copyrighted work claimed to have been infringed.
- Identification of the material claimed to be infringing.
- Contact information for the complainant, including mailing address, telephone number, and email address.
- A statement that the complaining party has a good faith belief that the use of the material is not authorized by the copyright owner, its agent, or the law.
- A statement that the information contained in the notice is accurate and, under penalty of perjury, that the complaining party is authorized to act on behalf of the copyright owner.
